Job Title: Family Care Nurse / In-home Nurse (Malaysia)

Work Location: Johor Bahru, Malaysia

Reporting To: Family Doctor / Head of Family Medical Services

Job Overview:

Provide professional and comprehensive nursing support for patients requiring in-home medical services by following doctors’ prescriptions and personalised care plans.

Responsibilities:

1.     Conduct a comprehensive health assessment based on the doctor’s prescriptions and patient needs.

2.     Develop and adjust personalised care plans to ensure compliance with local medical standards.

3.     Regularly measure and record the patient’s vital signs (e.g., blood pressure, temperature, heart rate).

4.     Monitor changes in the patient’s health status and promptly report any abnormalities.

5.     Implement medication management according to medical orders (including dispensing medication, injections, infusions, etc.).

6.     Clearly explain medication usage instructions and precautions to patients and their families.

7.     Responsible for wound disinfection, dressing changes, and follow-up checks.

8.     Provide initial first aid in emergency situations and coordinate patient referrals to hospitals as needed.

9.     Educate patients and their families on health knowledge, chronic disease management, and rehabilitation training.

10.  Offer guidance on lifestyle and nutrition to promote the patient’s overall health improvement.

11.  Maintain detailed nursing records to ensure data accuracy.

12.  Complete nursing documentation as required while adhering to data confidentiality and privacy regulations.

13.  Work closely with doctors to ensure seamless continuity of care.

Qualifications:

1.     Female candidates are preferred.

2.     Must hold a Malaysian Registered Nurse (RN) certificate and a valid practice license.

3.     At least 2 years of clinical nursing experience; experience in home care or community nursing is preferred.

4.     Solid foundational nursing knowledge and emergency care skills, with familiarity with local medical standards.

5.     Proficient in using home care equipment and electronic medical record systems.

6.     Good communication and coordination skills, with the ability to communicate in Chinese, English, etc.

7.     Strong emergency response skills and meticulous record-keeping habits.

8.     Ability to work flexible hours, including some weekends and night shifts.

9.     Understanding of local culture and the distribution of medical resources, with good regional adaptability.
